As we chatted about I will write you a private prescription for Flagyl for 10 days to treat the protozoa found. Ideally we should do the OAT test next before proceeding to the next phase of treatment to investigate fungal biomarkers as well as clostridia species biomarkers when you are able. In the meantime, start the candida diet and the spore forming probiotic 2 hrs away from the flagyl and continue this after finishing the antibiotics.

Clinician Message Ask your GP to do a CRP, ESR and get basic bloodwork if you haven’t had this recently eg. iron studies, B12 and folate levels, Vitamin D, complete blood count.
I would also consider another ketamine infusion with Dr. Dean Halfpenny in the future, depending on how you respond to cannabis. We will look the diet and supplements after you have done these tests. Before doing the stool test, come off of any probiotic pills for 3 weeks. If the other supplements you take have been stable for a long time, you can keep taking those up until 4 days before the other test but if some of them are new in the past month, stop them now and then do the test in approx 2-3 weeks time from when you stop. You can start the medical cannabis in the meantime that is not affected by the tests.

Clinician Message As we chatted about, I believe an integrative approach including cannabis based medicines as above would be helpful to improve many of your symptoms with a single medicine. However, because the amitriptyline is an NHS covered medicine, if you did wish to try that first, that would be another option for cost reasons. If you do decide you would like to proceed with the integrative plan and would like to start the cannabis based medicine, you can book the short specialist visit with Dr. Spavin to write the first prescription for you. AFter this, you would see me back in follow up subsequently. You do not have to do a full psychiatric hour long visit with her if you do not wish to, but since you have have not had any specialists involved previously, it may be a good idea to be most thorough, but it is totally up to you. If you would like to start with the amitriptyline, your nhs gp can prescribe that for you and I am happy to see you back to undertake the integrative part of your plan including the functional medicine tests, putting together a plan based on those results and also considering adding a device such as the CES device or NEurosym and working together to improve your symptoms over the next 306 months step by step. Also ask your GP for a referral to a pain clinic and they should also perform a baseline gp-level neurological exam looking for nerve impingements especially because I am not seeing you in person today so I cannot assess this.

As we chatted about, after xmas, lets start a detox
Calcium Ascorbate-take 1 tablet 2x/day starting now (this is the buffered vitamin C and it is from Nutrined)
During the detox after christmas period, go back on a Candida Diet for the entire 6 weeks. After the first 2 weeks, you can re-introduce some carbs such as non-potato root vegetables, small serving/day of quinoa or wild rice, one serving/day of lower sugar fruits but continue to avoid ‘free’ sugars and refined carbohydrates, bread and potatoes for 6 weeks if you can, the same with fermented foods for the whole 6 weeks if possible.
Colon cleansing: recommended at week 1, week 2 and then every other week for 6 week period. Aqualibria in London is reputable to have this done.
Then order the MOAT test 8 weeks after staring the cleanse.
Detox 3 step protocol using the Biocidin liquid, the Binder (GI detox +) and proflora spore forming bacteria in the new year. Take the Biocidin and the GI detox + Monday-Friday (eg. 5 days on) and then skip weekends (two days off) but take the Proflora probiotic every day.
f/u end of February or sooner if you have severe detox symptoms or side effects

keep on the magnesium, take in glycinate form at 600mg/day divided 300mg twice a day and try adding the Pharma Gaba 2 capsules/day and see how this works for the next month. Start a ‘reintroduction’ phase of testing each vegetable, eaten 2-3x/day for 2-3 days and if you don’t react, you can move that veggie into your ‘safe’ foods list so you can start having more veggies in the diet again. Also try minimizing anything with processed sugar including milk chocolate. 🙂

Clinician Message I do recommend getting a full thyroid panel done, this includes TSH, t3, rt3, t4, autoantibodies. Please upload your OAT test results. I think in the future future consider getting a urology consult for overactive bladder symptoms since this is impacting your sleep. We will start with a nighttime cannabis oil for your sleep and a daytime low dose of inhaled cannabis flower to see if this can help lift the mood and fatigue while we investigate gut and other contributors. I have recommended a cannabis flower strain that should be uplifting for mood and energy but it is also very high in THC. The lower THC ones are not as good for these two symptoms. Hence, start with just 1 2-second long inhalation, wait a good few minutes before taking a second inhalation if needed to avoid accidental overmedication especially because you are thc naive. you may just need 1 inhalation per time that you take it, and the effects generally last, after you have reached effect within the dosing session, for approx 2-3 hrs.

Diet: consider cutting out gluten for 6 weeks minimum trial to see if this impacts symptoms

Contact Prof Aziz’s secretary and get an appt if possible by video conference asap and send them results of all gi studies done so far. Let me know if he needs a referral note from me
Contact the cardiologist you saw at the heart institute locally and tell them POTs symptoms ahve wossened and yoiu need to see them again, say you have been in and out of hospital, very unwell and want to discuss further treatment for pots. If the waitlist is lengthy, I recommend seeing Dr. Nick Gall. If you do go this route, please send me via the clinic email a copy of the last cardiology letter you had and along with that I will do referral letter to Dr. Gall, as he normally does require one even for private patients.
Get your gp to refer you urgently back to UCL, since the local hospital has not been helpful in treating you on this or the previous admission. Tell the GP you are very worried and unable to tolerate anything including liquids by mouth. If you think it would be helpful, please email the clinic and I can write a letter back to your GP outlining the situation and the reason for the urgency from what I have seen. If you remember the consultant’s name who you were admitted under (was it GI there as well?) That will be helpful to tell the gp so they can better direct the referral.
Cannabis based medicines: start the daytime high cbd and the nighttime high THC oils as on the bottles after seeing Dr. Nij for the short visit. Then please book a follow up appt with me in 3 weeks after starting the medications or sooner if an issue with the medication arises, I think this will help you feel a bit better while we go through these other steps!
I also anonymously reviewed case with my neurologist colleague at our annual AGM for the non profit physician society which I am vice chair yesterday evening because he has some experience in these complex cases and he recommended Dr. Mark J Edwards in London. He is an expert in functional neurology who may be worth seeing for the functional seizures and overall neurological symptoms but first step is to see neurogastro. Clinician Message If the retreat is ok with it, start NAD+ injections once daily at the 0.25mls dose for week one, then once every other day at same dose for the next 3-4 weeks until our next meeting. If you have any side effects that persist after one week, stop injections until we meet again.

Also known as Nadi Shodhana, this alternate is a yogic breathing practice used for thousands of years in the yoga tradition to ‘balance’ brain hemispheres.Traditionally it was taught that regular dedicated practice over months can help ‘balance’ the mind.How To Do it:Place the tip of the index finger and middle finger of the right hand in between the eyebrows, the ring finger and little finger on the left nostril, and the thumb on the right nostril. We will use the ring finger and little finger to open or close the left nostril and thumb for the right nostril. If you are doing this in public and wish to be more discreet, you don’t have to place the index and middle finger on the forehead, just use a thumb to close off each nostril alternately!Press your thumb down on the right nostril and breathe out gently through the left nostril.Now breathe in from the left nostril and then press the left nostril gently with the ring finger and little finger. Removing the right thumb from the right nostril, breathe out from the right.Breathe in from the right nostril and exhale from the left. You have now completed one round of Nadi Shodhan pranayama. Continue inhaling and exhaling from alternate nostrils.Do 10 rounds by alternately breathing through each of the nostrils. After every exhalation, remember to breathe in from the same nostril from which you exhaled.Don’t force the breath, keep it natural and without force or effort. If you feel out of breath, stop and take a break until you feel the natural breath return.
